ADMISSIONS

ELIGIBILITY REQUIREMENTS FOR ADMISSION

Residents being admitted into the Forever Care Homes must meet the following entrance criteria:

  • Resident must have a Mentally Impaired diagnosis, as well as be an adult male or female aged 18 years and up

  • Voluntary consent by prospective resident and legal guardian

  • Prescreening assessment by licensee or designee​

  • Acceptable behavioral needs:

    • Agree to participate in treatment

    • Participate in assessment plan goals

    • Following the house rules

    • Follow medication schedule

  • Acceptable medical needs

    • Ambulatory

    • Diabetics-oral insulin dosages

    • Hypertension

    • Dementia
